Step-Down Mental Health Housing

Enhanced Support Service

At Bulwark Supported Living, we work closely with the NHS. We also collaborate with Southampton City Council and Hampshire County Council. Our goal is to support individuals transitioning back into independent living. Our service provides 12-week supported accommodation as part of a step-down pathway for those living with mental health needs. A short-term, community-based program designed to support individuals transitioning from hospital back into independent living.

A coaching session focused on developing essential life skills, confidence, and independence.

Our Approach to Step-Down Care for Mental Health Recovery:

Help early hospital discharge

Reduce inpatient stay on acute mental health wards

Allow safe and supported reintegration into the Southampton community

24-hour live-in care in fully furnished temporary accommodation

Working closely with CCO (Care Co-ordinators) and community mental and physical teams

Recovery-focused environment, assisting with daily living activities and social reintegration

Short-term placements for up to twelve weeks, with an emphasis on accelerated independence

Who is it for?

Bulwark Supported Living is a step-down service that provides support for patients who are clinically optimised to leave the inpatient ward, but waiting for non-clinical decisions or issues to be resolved. It for anyone who don’t meet acute level and can be referred by a community team.

What support is included

Bulwark Supported Living provides comprehensive support to help individuals regain independence after leaving inpatient care. Our team assists with daily living activities such as meal preparation and shopping, while also offering practical guidance on social care needs—including applications for housing, employment, benefits, and PIP. Residents benefit from a holistic, multi-disciplinary approach, including: Nursing support for mental well-being and medication management ✔ Occupational therapy assessments to improve daily functioning ✔ Activity coordination to enhance independent living skills ✔ Well-being sessions on mindfulness, relaxation, and structured 1:1 support ✔ Community referrals for volunteering, social groups, and addiction counselling
